By completing and signing this form, I confirm that I have had the full opportunity to ask any questions regarding my tattoo at DARWIN TATTOO INC. (INKNATION STUDIO). All of my questions have been answered to my satisfaction. I acknowledge that I have been informed of the following facts and details, and I agree to the terms outlined below.

1. Age Confirmation

I confirm that I have truthfully represented to Inknation Studio that I am over eighteen (18) years of age. *

2. Sanitary Compliance

I acknowledge and agree to follow any and all instructions provided to me regarding the maintenance of a sanitary environment before, during, and after my tattoo session. *

3. Allergic Reactions Disclaimer

I acknowledge that it is not reasonably possible for the representatives or employees of Inknation Studio to determine whether I may have an allergic reaction to the dyes, pigments, or processes used in my tattoo, and I accept the risk that such a reaction is possible. *

4. Voluntary Consent

I acknowledge that getting a tattoo is entirely my own decision, made of my own free will. I give my full consent to both the placement of the tattoo and the procedure being performed. *

5. Risk of Infection & Aftercare Responsibility

I acknowledge that there is always a risk of infection when receiving a tattoo, especially if proper aftercare is not followed. I confirm that I have received aftercare instructions and agree to follow them throughout the healing process. I understand that any touch-up work required due to my own negligence—including but not limited to damage caused by sun exposure or ultraviolet light—will be at my own expense. *

6. Medical & Skin Conditions

I acknowledge that I have informed my tattoo artist of any condition that could affect the healing of my tattoo. I confirm that I do not have any medical or skin conditions—including but not limited to acne, keloid scarring, eczema, psoriasis, freckles, moles, or sunburn—in the area to be tattooed that may interfere with the process. If I have any type of infection, rash, or irritation anywhere on my body, I will notify the tattoo artist prior to the procedure. *

7. Color & Design Variations

I acknowledge that there may be variations in both color and design between the tattoo I selected and how it ultimately appears on my skin. I understand that on darker skin tones, colors may not appear as bright as they would on lighter skin tones. I also recognize that over time, the colors and clarity of my tattoo may fade due to factors such as sun exposure and the natural dispersion of pigment beneath the skin. *

8. Permanence of Tattoos

I acknowledge that a tattoo is a permanent change to my appearance, and that no guarantees or representations have been made regarding the ability to alter or remove it in the future. To the best of my knowledge, I do not have any physical, mental, or medical condition that could affect my well-being as a direct or indirect result of my decision to receive a tattoo. *

9. Text in Tattoos

I acknowledge that if my tattoo includes text, I am solely responsible for verifying the correct spelling, grammar, and punctuation. I confirm that I have reviewed and approved the text prior to it being applied as a tattoo *

10. Symbols & Foreign Language Tattoos

I acknowledge that if my tattoo includes symbols or is written in a language other than English, neither the tattoo artist nor Inknation Studio is responsible for the meaning, translation, or interpretation of the text or design. *

11. Physical Reactions During Procedure

I agree to immediately notify the artist if I feel lightheaded, dizzy, or faint before, during, or after the procedure. I understand that failure to do so releases Inknation Studio and my tattoo artist from all responsibility. *

12. No Alcohol or Drugs

I confirm that I am not under the influence of alcohol or drugs at the time of this procedure. *

13. Medical Conditions & Pregnancy

I confirm that I do not have diabetes, epilepsy, hemophilia, a heart condition, or take blood-thinning medication. I do not have any medical or skin condition that could interfere with the application or healing of my tattoo. I am not the recipient of an organ or bone marrow transplant, or if I am, I have taken the prescribed preventive antibiotics as directed by my doctor prior to undergoing any invasive procedure such as tattooing. I am not pregnant or nursing, and I do not have any mental impairment that may affect my judgment in deciding to get a tattoo. *

14. Waiver of Liability

I agree to waive and release, to the fullest extent permitted by law, both the artist and Inknation Studio from all liability whatsoever for any and all claims or causes of action that I, my estate, heirs, executors, or assigns may have for personal injury or otherwise. This includes any direct and/or consequential damages resulting from the application of my tattoo, whether caused by the negligence or fault of the artist, Inknation Studio, or otherwise. *

15. Artistic Interpretation

I agree to allow the artist full creative and professional interpretation of the image being tattooed. *

16. Severability Clause

If any part of this agreement is found to be invalid or unenforceable, the remaining provisions shall remain in full effect. The invalid portion will simply be removed, and the rest of the agreement will continue as if it had never been included. *

17. Legal Fees & Jurisdiction

I agree to reimburse the artist and Inknation Studio (Darwin Tattoo Inc.) for any attorney’s fees and legal costs if I initiate legal action against them and they prevail. I further agree that any legal matters related to this agreement shall be resolved exclusively in the courts of New York County, New York. *

18. Photography Release

I give permission for any photos of my tattoo to be used by Inknation Studio in print or digital formats for promotional or educational purposes. *
